Time is myocardium. We have known this for decades. Perhaps the most compelling demonstration of this was shown by Weaver et al, 1 in the Myocardial Infarction Triage and Intervention (MITI) trial when it was demonstrated that fibrinolytic therapy initiated within 70 minutes of symptom onset was associated with a mortality of only 1.2%. We now know that all things being even, primary angioplasty is the preferred approach, but all things are rarely equal. Timely access to a catheterization laboratory and distance to a catheterization lab are two of the important things that are taken into account when deciding whether to treat a STEMI patient with primary angioplasty or fibrinolytic therapy.
